Present Perfect =    have / has + participe passé

Remarque:

Phrases 3 et 11:  on peut répondre de deux façons en fonction de ce que l'on veut exprimer :

to go = aller

She has gone to Peru = elle est partie au Pérou ( et elle n'est pas revenue )

she has been  to Peru = elle est allée au Pérou ( mais elle est revenue )
